you may also be required to provide third-party health portal credentials
(“Portal Credentials”) that allow us to retrieve your health data from external
healthcare providers (“Portal Information”). This data is collected, stored,
and processed solely for your use and benefit.
Access to
your Registration Information, Portal Credentials, Portal Information, and any
other personal data you provide is protected by internal safeguards and
procedures designed to restrict access and ensure that your information is used
only to operate, develop, and improve the Services.
The personal
information we collect may include the following:

Sensitive
Information. We do not collect sensitive personal information directly. However, any

information may be included in the medical records you provide or authorize us
to access. This processing is conducted solely to deliver our Services and is
subject to your consent or applicable legal permissions. The sensitive
information we collect from the medical records may include the following:

■ Health or biometric data, including diagnoses, treatments, vaccination records communicable diseases (e.g., HIV, Hepatitis), addiction or substance abuse treatment, psychotherapy notes, and mental health records

■ Political opinions

Payment Data. We may collect data necessary to process your payment if you make
purchases, such as your payment instrument number (such as a credit card
number), and the security code associated with your payment instrument. All

Social Media Login Data. We may provide you with the option to register with us using your
existing social media account details, like your Facebook, Twitter, or other
social media account. If you choose to register in this way, we will collect
the information described in the section called "HOW DO WE HANDLE YOUR SOCIAL
LOGINS?" below.
Application Data. If you use our application(s), we
also may collect the following information if you choose to provide us with
access or permission:

Geolocation Information. We may request access or permission
to track location-based information from your mobile device, either
continuously or while you are using our mobile application(s), to provide
certain location-based services. If you wish to change our access or permissions,
you may do so in your device's settings.
■ Mobile Device Access. We may request access or permission
to certain features from your mobile device, including your mobile device's bluetooth, calendar, camera, own, and other features. If
you wish to change our access or permissions, you may do so in your device's

Mobile Device Data. We automatically collect device
information (such as your mobile device ID, model, and manufacturer), operating
system, version information and system configuration information, device and
application identification numbers, browser type and version, hardware model
Internet service provider and/or mobile carrier, and Internet Protocol (IP)
address (or proxy server). If you are using our application(s), we may also
collect information about the phone network associated with your mobile device,
your mobile device’s operating system or platform, the type of mobile device
you use, your mobile device’s unique device ID, and information about the
features of our application(s) you accessed.
■ Push Notifications. We may request to send you push
notifications regarding your account or certain features of the application(s).
If you wish to opt out from receiving these types of communications, you may
turn them off in your device's settings.
This information is primarily needed to maintain the
security and operation of our application(s), for troubleshooting, and for our
internal analytics and reporting purposes.
All personal
information that you provide to us must be true, complete, and accurate, and
you must notify us of any changes to such personal information.

(e.g., Facebook or Twitter), we receive personal information about you such as
your name, email address, and gender. Any personal information that we collect
from your social media account depends on your social media account's privacy

■ When we use Google Maps Platform APIs. We may share your information with certain Google Maps Platform APIs (e.g., Google Maps API, Places API). This includes: device GPS coordinates, approximate geolocation data (via IP address), and location-based search queries, and other similar information. 8. IS YOUR
INFORMATION TRANSFERRED INTERNATIONALLY?

Our servers are located in United States. If you are

Commission's Standard Contractual Clauses:
We have
implemented measures to protect your personal information, including by using
the European Commission's Standard Contractual Clauses for transfers of
personal information between our group companies and between us and our
third-party providers. These clauses require all recipients to protect all
personal information that they process originating from the EEA or UK in
accordance with European data protection laws and regulations. Our Data
Processing Agreements include Standard Contractual Clauses. We have implemented
similar appropriate safeguards with our third-party service providers and
partners and further details can be provided upon request.
9. HOW LONG DO WE
KEEP YOUR INFORMATION?
In Short: We keep your
information for as long as necessary to fulfill the purposes outlined in this
privacy notice unless otherwise required by law.
We will only
keep your personal information for as long as it is necessary for the purposes
set out in this privacy notice, unless a longer retention period is required or
permitted by law (such as tax, accounting, or other legal requirements).
- Inactive
Accounts: If your
account is unused for an extended period, we may suspend or “lock down”
your account to better safeguard your personal information. We will notify
you when this occurs. If your account remains inactive, we will retain
your personal information for two (2) years following
suspension. After that period, we will delete your personal information to
protect your privacy. Prior to deletion, we will attempt to notify you.

Accounts: If your
account is closed by you or terminated by us in accordance with our Terms
of Service, we will promptly delete the personal information in your
account, except for a disclosure log that records how you
shared your information with third parties. This log will be retained only
as long as necessary for our business purposes and deleted as soon as
practicable under our record retention policies.
- Deletion
Requests: You may
request deletion of your personal information at any time by contacting us
at privacy at mymedicalrecords.ai. Upon such request, all
personal information associated with your account will be deleted
immediately and will no longer be accessible by you or My Medical Records.
The only exception is the disclosure log, which we retain for audit and
legal purposes.
Please note that while we make reasonable efforts to comply with
deletion requests, complete and comprehensive removal of data from all systems
(e.g., backups) may not be guaranteed.
In that case, we will securely store your personal
information and isolate it from any further processing until deletion is
possible.

In Short: We aim to protect
your personal information through a system of organizational and technical
security measures.
We have
implemented appropriate and reasonable technical and organizational security
measures designed to protect the security of any personal information we
process. However, despite our safeguards and efforts to secure your
information, no electronic transmission over the Internet or information
storage technology can be guaranteed to be 100% secure, so we cannot promise or
guarantee that hackers, cybercriminals, or other unauthorized third parties
will not be able to defeat our security and improperly collect, access, steal,
or modify your information. Although we will do our best to protect your
personal information, transmission of personal information to and from our
Services is at your own risk. You should only access the Services within a
secure environment.

13. CONTROLS FOR
DO-NOT-TRACK FEATURES
Most web
browsers and some mobile operating systems and mobile applications include a
Do-Not-Track ("DNT") feature or setting you can activate to signal
your privacy preference not to have data about your online browsing activities
monitored and collected. At this stage no uniform technology standard for
recognizing and implementing DNT signals has been finalized. As such, we do not
currently respond to DNT browser signals or any other mechanism that
automatically communicates your choice not to be tracked online. If a standard
for online tracking is adopted that we must follow in the future, we will
inform you about that practice in a revised version of this privacy notice.

Information that is collected or processed without the purpose of inferring
characteristics about a consumer is not covered by this right, as well as the

Verification
process
Upon
receiving your request, we will need to verify your identity to determine you
are the same person about whom we have the information in our system. These
verification efforts require us to ask you to provide information so that we
can match it with information you have previously provided us. For instance,
depending on the type of request you submit, we may ask you to provide certain
information so that we can match the information you provide with the
information we already have on file, or we may contact you through a
communication method (e.g., phone or email) that you have previously provided
to us. We may also use other verification methods as the circumstances dictate.
We will only
use personal information provided in your request to verify your identity or
authority to make the request. To the extent possible, we will avoid requesting
additional information from you for the purposes of verification. However, if
we cannot verify your identity from the information already maintained by us,
we may request that you provide additional information for the purposes of
verifying your identity and for security or fraud-prevention purposes. We will
delete such additionally provided information as soon as we finish verifying
you.
Other
privacy rights
■ You may object to the processing of your personal information.
■ You may request correction of your personal data if it is incorrect or no longer relevant, or ask to restrict the processing of the information.

Incentives
"Financial
incentive" means a program, benefit, or other offering, including payments

permits financial incentives or a price or service difference if it is
reasonably related to the value of the consumer’s data. A business must be able
to explain how the financial incentive or price or service difference is
reasonably related to the value of the consumer’s data. The explanation must
include:
■ a good-faith estimate of the value of the consumer’s data that forms the basis for offering the financial incentive or price or service difference; and
■ a description of the method the business used to calculate the value of the consumer’s data.
We may

process
We may

you and your consumer's request. If you submit the request through an
authorized agent, we may need to collect additional information to verify your
identity before processing your request.
Upon
receiving your request, we will respond without undue delay, but in all cases,
within forty-five (45) days of receipt.
The response period may be extended once by forty-five (45) additional
days when reasonably necessary. We will
inform you of any such extension within the initial 45-day response period,
together with the reason for the extension.
Right to
appeal
If we
decline to take action regarding your request, we will
inform you of our decision and reasoning behind it. If you wish to appeal our

sixty (60) days of receipt of an appeal, we will inform you in writing of any
